Who is the Care Journal for?
The Care Journal is for anyone supporting a parent, partner, relative or friend. It gives visits, what you noticed and practical follow-up one place, whether one person uses it or several people share it. The review and appointment pages can also help organise dates, changes and questions before speaking with a GP or another professional.
Why not use a normal notebook?
For some people, a normal notebook works well. The Care Journal saves you from rewriting the same headings at every visit and gives everyone a consistent set of prompts, review pages and appointment notes. This can make entries quicker to complete and easier to compare, find and follow up.
What can I record?
The journal covers six core areas: mood and engagement, energy and alertness, communication, movement and independence, eating and drinking, and comfort. You can also choose two optional focus areas of your own, based on what matters to the person and their care.
Do I have to fill in everything?
No. The journal is designed to support the way you care, not add more work. Use “not observed” or leave prompts blank, choose the focus areas that matter and record only what is useful for that visit.
How many visits does it cover?
The current design includes 36 visit records, review sections and dedicated appointment or conversation notes.
Is there enough room for notes?
Each visit combines quick prompts with dedicated writing space for what was different, what may have happened beforehand, what appeared to help and notes for next time.
Can several people use it?
Yes. Agree who may use it and where it will stay. Those people can add their own notes. The person being supported should be comfortable with what is written and who can read it.
Can I take it to a GP or other appointment?
Yes. Use the review and appointment pages to remember dates, changes and questions you want to raise. A professional decides what information is relevant.
Is it a medical record?
No. It is for personal organisation and note-taking. It does not replace professional records, instructions, medical advice or emergency help.
Is it only for dementia care?
No. The first Care Journal is general-purpose. Its baseline and optional focus areas allow the prompts to be adapted to the person being supported.
How much does it cost?
One journal is £12.99 and the two-journal pack is £19.99. Delivery to England, Scotland and Wales is free.
When will it be dispatched?
Orders will be dispatched from 1 October 2026 and delivered by 8 October 2026

We currently deliver to England, Scotland and Wales. We do not currently deliver to Northern Ireland, the Channel Islands, the Isle of Man or international destinations.
